Terms of service

The agreement between you and Contin-U Medical. Effective September 20, 2026.

Who we are

CONTIN-U LLC (a Florida limited liability company, doing business as Contin-U Medical) operates the contin-u.com platform, handles all billing, and provides non-clinical services. All clinical services are provided by physicians licensed in the patient's state of residence. We refer to CONTIN-U LLC as “Contin-U,” “we,” or “us.” Our principal address is 6421 N Florida Ave, D-1616, Tampa, FL 33604.

1. Acceptance

By accessing this website or our services, you agree to these Terms. If you do not agree, do not use the site or our services.

2. Service area

Clinical services are available to U.S. residents 25 years of age or older who reside in a state where Contin-U has a licensed prescribing physician and a pharmacy licensed to dispense. As of the Last Updated date above, that footprint is Florida and Massachusetts. This footprint may change; the current list is shown during the eligibility check. We reserve the right to verify residency and decline service to anyone we cannot confirm meets these requirements.

3. No physician-patient relationship from website use

Browsing this site or completing the eligibility check does not create a physician-patient relationship. A physician-patient relationship is established only after you complete a full intake, our medical team accepts your case, and you have your initial telehealth visit.

4. Membership, billing, and payment

4.1 One membership. Contin-U offers a single service: physician-supervised testosterone therapy, billed as a membership of $125 per month. There are no tiers, no prepaid terms, and no long-term contract.

4.2 What you pay at signup. Your first charge is $250, which covers your first two months of membership. We bill the first two months together at signup so that your first payment and your first shipment align. Your prescribed medication is included in your membership. The quantity dispensed and how long it lasts are determined by the dose your physician prescribes for you. If you do not have bloodwork from the last six months, a $99 baseline hormone panel is added at checkout, making the signup total $349. If you already have recent labs, there is no charge to have a physician review them.

4.3 Intake and physician review fee.

(a) The fee. Contin-U charges a one-time intake and physician review fee of $115. This fee covers the clinical work performed before any prescription is written: review of your intake questionnaire and medical history, physician interpretation of your laboratory results, the eligibility determination itself, and the documentation required to support it. It is separate from your membership fee and from any laboratory fee.

(b) The fee is currently waived for new patients. As a standing promotion, Contin-U waives the $115 intake and physician review fee in full for every new patient enrolling in a membership. You are charged $0 for intake and physician review at signup. This waiver has no announced end date. Contin-U may discontinue it for future enrollments at any time by updating these Terms and the pricing shown at checkout. Discontinuing the waiver will never create a charge for a patient who already enrolled while it was in effect.

(c) When the fee is charged. The fee is charged, and is not waived, in the following circumstances:

  • Re-enrollment. If you cancel your membership and later re-enroll more than 90 days after cancellation, a new intake and physician review is required and the $115 fee applies. Re-enrollment within 90 days does not incur the fee.
  • Repeat evaluation after a declined determination. If a Contin-U physician determines that testosterone therapy is not clinically appropriate for you and you later submit a new application with new laboratory results, the second review is charged at $115.

In either case, the fee is disclosed to you at checkout before you authorize payment, and it is a one-time charge that does not recur and does not enroll you in any subscription.

(d) Refundability. Where the $115 fee has been charged and the physician review has been performed, the fee is not refundable, because the clinical work was completed. Where the fee has been charged but no physician review has yet occurred, it is refunded in full on cancellation.

4.4 Recurring billing after the first two months. Beginning in the third month, your card on file is charged $125 on the same day of each month until you cancel. By enrolling you authorize CONTIN-U LLC to make these recurring charges. We email you a receipt for every charge. There is no enrollment fee and no charge for shipping. The intake and physician review fee described in Section 4.3 is waived for new patients.

4.5 No minimum commitment. Cancel at any time. There is no minimum term and no notice period. You may cancel at any time, effective immediately, using the online cancellation form, from your member dashboard once it is available, or by emailing support@contin-u.com. Cancellation stops all future charges. The two months collected at signup are not prorated or refunded once a prescription has been written and shipped, because medication has been dispensed to you. Amounts already billed for months in which medication was dispensed are not refunded except as described in Section 4.6 or as required by law.

4.6 If treatment is not clinically indicated. If, after your laboratory evaluation and physician review, a Contin-U physician determines that testosterone therapy is not appropriate for you, the membership amount you actually paid at signup is refunded in full within 10 business days — $250, or the discounted amount if a promotional code was applied. The $99 laboratory fee is not refundable once the panel has been drawn and resulted, because the laboratory work was performed. This refund applies only where treatment is declined by a Contin-U physician for clinical reasons, and does not apply after a prescription has been written.

4.7 Laboratory fees. The $99 baseline hormone panel is a one-time charge, not a subscription. It does not recur and does not auto-renew. Follow-up monitoring labs required during therapy are included in your membership at no additional charge.

4.8 Two billing streams. Your clinical membership fee (physician evaluation, ongoing oversight, care coordination, and platform access) is billed by CONTIN-U LLC under merchant category code 8099 (Health Services). Where medications are dispensed through our contracted 503A pharmacy partner, the medication charge is billed separately by CONTIN-U LLC under merchant category code 5912 (Drug Stores and Pharmacies). Both descriptors identify “CONTIN-U MEDICAL” on your card statement.

4.9 Merchant of Record. CONTIN-U LLC is the merchant of record for all charges appearing on your card statement, including both clinical membership fees and, where applicable, medication charges dispensed through our contracted pharmacy partners.

4.10 Non-returnable prescription products. Once a prescription medication has been dispensed and shipped by our pharmacy partner, that medication is not eligible for return or refund. This is standard practice for prescription products and is required by state pharmacy law.

4.11 Membership does not guarantee a prescription. Treatment decisions are made solely by your physician based on clinical evaluation. Enrollment and payment do not entitle you to a specific medication, dose, or protocol.

4.12 Promotional credits. Referral credits are governed by our Refer a Friend program terms, which are incorporated into these Terms by reference.

4.13 Promotional codes. Contin-U may offer promotional codes that reduce the amount charged at signup. Unless the code’s terms state otherwise: a code applies only to the membership portion of your signup charge and not to any laboratory fee; it applies only to the initial charge and does not reduce your recurring $125 monthly membership fee; only one code may be applied per enrollment; and codes have no cash value, cannot be applied retroactively to a completed purchase, and may be modified or discontinued at any time. Where a promotional code has reduced your signup charge, any refund owed under Section 4.6 is calculated on the amount you actually paid.
